    • When: June 8, 2022
    • Where: Crest Hollow Country Club
    • Time: 6pm-11pm
    • Price: $90
 
PROM IS APPROACHING! We are going to be collecting money from Monday May 9th to Friday, May 27th. 
 
In the main office, you can pick up the Prom Information Packet. In this packet, you will find three sheets of paper: 
  • First Page: Maspeth High School 2022 Prom Contract
    • This contract must be signed by the student attending and the student's parent/ guardian
    • This contract must be completed as well as proof of vaccination at time of payment
    • If a senior is buying a ticket for an underclassman, the underclassman must also complete this form.
    • Please read through this form in its entirety before signing.

  • Second Page: Maspeth High School Outside Guest Form 
    • This form is only for students that are bringing a non-Maspeth guest to prom.
    • This form must be filled out at time of payment 
    • Photo ID and proof of vaccination must be provided for the guest. This can either be photocopied or emailed to Ms. Koskinas and Mrs. Healy.
    • Guest form must be signed by guest attending and parent/ guardian of guest. 
    • Guest cannot be over the age of 20.
  • Third Page: Prom Information 
    • This page is for you to KEEP. It has all the information for prom.

    • Cost: $90 per ticket 
We will be collecting money before school, after school, and during 7th period in room 312.
