COVID Testing Consent

The New York City Schools Chancellor announced on May 24, 2021, that “On-site COVID-19 testing will continue in school buildings as recommended by the latest health guidance.”

 

We ask all parents/guardians, who have NOT already done so, to complete the COVID Testing Consent form for your child.
